The SCCP is a partnership project between Allina Health Care System and Elderberry Institute. It's aimed at creating a new model of improved care for elderly people by more closely integrating the LAH/BNP model of community based services and support with current medical delivery systems. A typical participant is a woman in her 80's living alone with multiple support needs.

    The project establishes an infrastructure providing continuity of support services for an elder during all transitions into and out of the clinic, hospital, rehabilitation center, nursing home and back home again. The partnership objective is to benefit elders they serve by strengthening the cooperative infrastructure and improving communication between all partners.

    Partners will provide more seamless transitions for elders served by making changes as appropriate in their communications methods and care coordination structures. The clinic and the hospital will provide medical care and the local LAH/BNPs will provide a range of volunteer and paid support services (for example, accompanying elders to appointments, helping elders pre-plan what needs to be discussed with their doctor, etc.). An active participant since the project's inception, the Allina Foundation made an initial commitment to fund the evaluation component and has provided leadership in seeking other partners.
