Foundation Funding History for the Living At Home and Block Nurse Programs
This funding history includes outcomes where LAH/BNP, Inc. was actively involved in the solicitation process and shows our active involvements with foundations. It does not include all of the fundraising activities undertaken by local community Programs.
- 1982 First among many local foundation grants to the St. Anthony Park BNP was the H.B. Fuller Company.
- 1984 Funding from the St. Paul Foundation for an evaluation of the pilot in St. Anthony Park.
- 1986 Local foundations and St. Paul NPP provided grants to the Highland BNP.
- 1987 Based upon a grant proposal prepared by the BNP, Inc. the W.K. Kellogg awarded $400,740 to the Highland BNP and $297,029 to the Atwater Area BNP for a three year replication project. It also granted $411,330 to the BNP, Inc. for operations and evaluation of the replication over three years. (about $175,000 was spent on evaluation).
- 1987 Based upon a grant proposal from the BNP, Inc., the Division of Nursing, U.S. Department of Health and Human Services granted North End-South Como BNP $324,740 for three years and the BNP, Inc. for operations and evaluation for three years, $246,144.
- 1987 The Living at Home Programs in Macalester/Groveland and West 7th Street communities were funded with a $350,000 grant from the Commonwealth Fund, the St. Paul Foundation and the St. Paul Companies.
- 1988 The Prospect Park BNP secured funding from local foundations.
- 1989 The Living At Home Program and Block Nurse Program Collaborative effort received $25,000 from the St. Paul Foundation to develop a combined model, collect and analyze data from all sites and prepare a comparative report on socio-demographic characteristics, use of services and cost of the program.
- 1990 Merriam Park received $14,800 NPP money.
- 1991 The BNP, Inc. sent rationale to the W.K. Kellogg Foundation to extend the project for 2 more years in Highland Park and Atwater since the projected number of clients had not been enrolled resulting in dollars not expended.
- 1991 The BNP, Inc, was funded for 1992-1993 by:
- Saint Paul Foundation $ 20,000
- Metro Council $ 27,000
- Ramsey County $ 10,000
- Bush Foundation $ 142,956
- Blandin Foundation $ 142,956
- 1991 The Kellogg Foundation granted a $85,000 Dissemination Grant to the BNP, Inc. Included was money for educational modules for communities, boards, staff and volunteers, a national conference, etc.
- 1992-3 Many of the 14 Living at Home/Block Nurse Program sites have raised money and written grant proposals and have been awarded funding estimated to be over $100,000.
- 1993 The Bush Foundation granted $37,650 to LAH/BNP, Inc. in order to cover budget obligations for 1993.
- 1994 The Living at Home/Block Nurse Program, Inc. received a grant of $141,395 from the Bush Foundation and $160,000 from the Blandin Foundation.
- 1994 HCFA and LAH/BNP, Inc. decided to proceed with the three year CNO demonstration; a grant of $911,802 was awarded to develop the four communities, contribute to research, etc.
- 1995 The Blandin Foundation funded LAH/BNP, Inc. with $120,000, the Bush Foundation with $149,832.
- 1995 The communities independently raised approximately $200,000.
- 1995 LAH/BNP, Inc. wrote proposals on behalf of all Programs that netted $89,000 for the communities for client services.
- 1996 The following foundation grants were given to LAH/BNP, Inc.:
- Bush granted $75,000 for 1996 and $50,000 for 1997
- Medtronic granted $15,000 for 1996 and $10,000 for 1997
- Blandin granted $80,000 for 1996-1998 and a $70,000 matching grant for establishing managed care relationships with rural sites
- H.B. Fuller granted $5,000
- Deluxe granted $5,000
- 1997 The following foundations awarded grants to LAH/BNP, Inc.:
- 3M Foundation awarded $10,000 for a new Volunteer Manual.
- Land O'Lakes Foundation awarded $10,000 for Communications Link to Seniors Project.
- Beim Foundation awarded $10,000 for Communications Link to Seniors Project.
- Otto Bremer Foundation awarded $10,000 for Communications Link to Seniors Project.
- MN Mutual Foundation awarded $5,000 for Communications Link to Seniors Project.
- Deluxe Corporation Foundation awarded $5,000 for general operations.
- Union Pacific Foundation awarded $5,000 for general operations.
- Medtronic Foundation awarded $10,000 for low income/minority project.
- 1999 The following Foundations awarded grants to LAH/BNP, Inc.:
- Beim Foundation awarded $8,600 for a new local area network server in preparation for Y2K.
- Allina Foundation awarded $19,000 for general operations.
- The Public Welfare Foundation awarded $75,000 for the Senior Care Community Partnership ($25,000) and Elderberry Institute ($50,000).
- The Minnesota Mutual Foundation awarded $4,000 for general operations.
- The Cy Press Fund awarded $28,168 for the Senior Care Community Partnership ($15,000) and general operations ($3,168).
- The Bush Foundation awarded $117,000 for Elderberry Institute ($30,000 seed fund - $87,000 core development).
- The Retirement Research Foundation awarded $100,000 over three years to Elderberry Institute. ($50,000 per year seed fund; $50,000 per year core development).
- -HealthEast Communities Fund awarded $5,000 for general operations.
- -Charlson Foundation awarded $5,000 for general operations.
- 2000 The following Foundations awarded grants to LAH/BNP, Inc.:
- The Bush Foundation awarded $260,000 over two years for Elderberry Institute ($50,000 per year for seed fund; $80,000 per year for core development).
- Minnesota Mutual Foundation award $4,000 for general operations.
- Blandin Foundation agreed to transfer $70,000 awarded in 1996 to the Elderberry Institute seed fund.
